REGISTRAR & ENROLMENT SERVICES
This form is used to request an update to your name or date of birth on file. Updates to your mailing address, email address, or telephone
number must be updated through Langara’s login page. Please note, if you are or were previously an employee of Langara College you
should not use this form. If this applies to you, please contact HR for guidance.
DOCUMENTS REQUIRED
Acceptable documentation includes, but is not limited to, the following: government issued ID (eg. passport or driver’s license), birth
certificate, certificate of change of name, marriage certificate, divorce certificate, and notarized leer. Photocopies will not be accepted
unless they are notarized. The Records department may request additional documentation.
PREFERRED FIRST NAME CHANGES
Preferred first names appear on instructor class lists and the Brightspace online platform. In order to add a preferred first name onto
your student record, fill out the ‘Preferred First Name Change’ option below. No supporting documentation is required in order to add a
preferred first name to the student record. Any official documentation (ie. Transcripts, credentials etc.) produced by Langara College will
only include legal names.
