CONSENT & RELEASE
(FOR CONTEST PARTICIPATION)
By signing below, I, _____________________________________________, represent and warrant that I am an adult and the
parent or legal guardian of _____________________________________________ (the “Minor”), and that I have all necessary
rights and legal authority to enter into this Consent and Release (the “Release”). I hereby grant my consent to
the Minor’s participation in ___________________________________________________________________________ (the “Contest”).
As consideration for the Minor’s right to participate in the Contest, on behalf of myself and the Minor
(including any others who may assert any claims in the Minor’s name), I agree as follows:
• I have read the Official Contest Rules, located at easternflorida.edu/promotions (the “Contest Rules”)
and understand that the Minor’s participation in the Contest constitutes his or her and my full and
unconditional agreement to and acceptance of the Contest Rules and the decisions of Eastern Florida
State College (the “Sponsor”), which are final and binding.
• If the Minor is selected as a winner, he or she may accept a prize (of which I shall be responsible for
any resulting tax obligations), and the Sponsor has the right to use his or her name and likeness in
any promotion or marketing related to the Contest, without further compensation to the Minor or
myself.
• I expressly waive, release, hold harmless and forever discharge the District Board of Trustees of
Eastern Florida State College, Florida; its affiliated or related entities; and its respective individual
trustees, officers, employees, agents, volunteers, contractors, successors and assigns, as well as any
other entities associated with the Contest and each of their respective officers, directors, employees,
shareholders, representatives, and agents from and against any claims or causes of action arising out
of participation in the Contest or receipt or use of any prize (including any travel or incidental
activities related thereto).
• I agree that this Release is governed exclusively by Florida law, and that any action(s) arising out of
this Release and/or the Contest shall be brought in state court in Brevard County, Florida.
