Please list all REQUIRED books needed for the part of term. You must provide documentation of the cost of the
books. A printout from the Bookstore’s website is acceptable.
E. Certification and Signature
• I understand that I am financially responsible for all purchases incurred at the EFSC Bookstore.
• I understand that the cost of all materials purchased at the Bookstore via a book voucher will be deducted
from my financial aid before refunds are made.
• I understand that dropping classes, withdrawing from classes, and not attending classes can impact my
eligibility for aid and may result in a balance due to EFSC.
