On-Line Perming User Declaraon
Hillsborough County Development Services- Building Services Division requires that the license qualier read the
following legal declaraons. The qualier will need to complete all required informaon, sign the form and email it to
perming@hcgov.net.
On-Line Perming (a.k.a. VelocityHall) is an oponal internet service provided by Accela, Inc. and Hillsborough County,
Florida. The use of this service is at my discreon. This internet service is provided without addional convenience fees or
merchant account fees charged by Accela or the Hillsborough County to me. The permit fees associated with the issuance of
a permit are the same that would be assessed if I obtained the permit directly from the Hillsborough County Development
Services - Building Services Division. Should a refund be required, Hillsborough County will issue a check in the name of the
qualier listed below or the Business name as registered with the Building Services Division. A charge back to the user’s credit
card is not available.
General Condions:
On-Line Perming permits are available for those permits that do not require any discreonary approvals or plan review by
Building Services. If, subsequent to the issuance of an On-Line permit, the Building Ocial determines that plans or other
reviews are required for the work undertaken, the Building Ocial may revoke the permit or suspend the permit unl such
me as proper reviews and approvals are obtained. As the Applicant, I understand I will be informed of any such revocaon
or suspension.
I understand that the issuance of an On-Line permit shall not serve to legalize any other unpermied construcon not properly
idened within the permit applicaon, improper or unauthorized land uses not otherwise legally established. All On-Line
Permits are subject to review and approval by Hillsborough County Development Services - Building Services Division.
All work authorized by an On-Line permit must comply with all applicable state and local statutes, ordinances, rules,
regulaons, and codes. All work authorized by a permit shall be inspected. Inspecons can be requested online or by calling
the Hillsborough County Interacve Voice Response System (IVRS). Inspecon scheduling numbers are as follows: (813)272-
5613, (813)272-5618, or (813)272-5639.
Access must be provided for inspecon purposes.
Permits shall automacally EXPIRE and become void if work is not commenced within 180 days following the date of
issuance, or if work is suspended or an inspecon is not approved within 180 days from the date of issuance.
By applying for a permit, I arm that at the me of making applicaon and unl nal inspecon and closure of permit, that I
have and will maintain a Cercate of Workers Compensaon Insurance or Worker’s Compensaon Exempon, a Cercate
of General Liability Insurance, a valid and current Cercate of Competency license issued by either the State of Florida or
Hillsborough County, as required by state and local laws, and the provisions of Chapter 489 of Florida Statute; and any and all
other applicable licenses.
I cerfy that I have read the informaon and condions of my receiving and issuance of permits using the On-Line Perming
system. I agree to comply with all applicable state and local statues, ordinance, rules, regulaons, and codes relang to
construcon, and authorize representaves of Hillsborough County Building Services Division, to enter upon the property for
inspecons of the permits for which I have obtained.
