Attach a site plan showing all structures and parking. Donation trailer must be located in an area that will not imped the flow of traffic.
Failure to include any of the items below will result in your application being deemed incomplete.

Site Plan showing placement of donation trailer relative to
existing structures and parking on-site.
Photo or drawing of the side of the proposed trailer showing that the
organization's name, address, and phone number are clearly marked
on the trailer.
There is no fee for this permit, but an application is required for each donation trailer.
