To participate in the HomeWorks upgrade program:
1. Submit this completed form along with:
a. Your purchase order for the new equipment you need
b. A list of old equipment that you will be returning
2. Lutron will issue you an RGA (Returned Goods Authorisation) for the equipment you wish to return.
3. After you have replaced the old equipment with the new HomeWorks QS equipment, return the old
equipment to Lutron marked with the RGA number given to you by Lutron.
4. Old equipment can be returned up to 90 days after the new equipment has been sent to you.
