1. Owner Recitals and Covenants
As owner(s) of the above-identified property, by signing this agreement, I acknowledge and agree to the
following recitals and covenants:
A. The undersigned requests a single water service tap-in or access point to the owner(s) property identified above. The
work to be performed to connect to the Canton City water system shall be performed and completed by the owner(s)
at the owner(s) expense. It shall also be in conformity with all applicable city ordinances, rules, regulations, policies and
conditions as established by the City and/or its related departments, including any modifications or amendments of the same,
which may be made from time to time.
B. Owner(s) further agree they will:
1. be liable for timely payment for the water usage charges to the above property,
2. be responsible for any damage to water meter(s),
3. be charged $500 for any tampering or damage done in relation to the water meter, meter transmitting unit,
and shut off valve/curb box at his or her property. Remedies are cumulative. Each item named shall
constitute a sperate charge for the above noted fee. Property owners who believe they have been charged
this fee in error may appeal to the Utility Accounts Appeals Board.
4. ensure that the plumbing installed at the above premises shall be performed by plumbers licensed by the
City of Canton, (subject to the Homeowner’s Exemption provision of Section 1315.07), and shall meet the
standards of the Plumbing Code of the codified ordinances of the City of Canton (Chapter 1315),
5. ensure that inspections of plumbing installed shall be performed by certified plumbing inspectors employed
by the City of Canton and an appropriate fee schedule for said inspections shall be paid, and,
6. comply with the Canton Water Department Rules and Regulations for Water Charges and Bills (current or as
amended and acknowledge receipt of a copy of same. ________INITIAL
7. be liable for loss or excessive damage to 95-gallon sanitation cart and comply with Sanitation
guidelines.
C. In the event that owner shall violate any of the terms and conditions of this agreement or any of the ordinance, rules,
regulations, policies and conditions of the city, pertaining to the city water lines and water services, owner(s) agrees that the
city shall have the authority to shut off water services to owner(s) property.
D. It is understood and agreed that all charges, which are unpaid and become delinquent, may be certified to the county
auditor to be placed upon the tax duplicate and collected as an assessment against the property and owner(s), in addition to
any other remedies or methods of collection permitted by law.
E. Owner(s) agree that if any of the information provided herein should change, such as phone number or
residence address, it is the responsibility of owner(s) to provide updated and current information to the Canton City
Water Department.
