Pet Owner Agreement
In order to ensure a safe, healthy and positive living environment for all residents in our community,
you have sole responsibility for the care, control and supervision of your pet. By reading and
signing this document, you indicate you are aware of this responsibility.

Please review the following statements and sign below.
1. I understand and agree that I am permitted to have one pet in the Johnson & Wales Pet-Friendly Community and it can
only be the pet identified on the submitted application.
2. I understand and agree that the university is not responsible, in any manner, for my pet.
3. I understand and agree that my pet will be fully licensed in accordance with city ordinance prior to me bringing it on to
campus and that I have provided accurate proof of all required vaccinations if necessary. Additionally, I understand and
agree that if my pet is a dog or cat, it must be spayed or neutered and documentation of that procedure must be
submitted to Residential Life prior to my pet residing on campus. I understand and agree no pregnant pets are allowed on
campus.

I have thoroughly read and agree to the Pet-Friendly Community Guide 2017-2018.
5. I understand and agree that I will notify my roommate(s) that a pet will be residing in our room/suite/apartment.
Residential Life will also complete a follow-up to ensure all residents in the space are aware and in agreement with the
presence of the pet in the shared space. I understand that my roommates and I will be expected to complete a
roommate agreement, which will be kept on file with Residential Life.
6. I understand and agree that my pet is restricted to my residence hall room/suite/apartment and is not permitted to
roam outside of my room/suite/apartment, within the residence hall (including common areas), in other student rooms/
suites/apartments, or in any other building on campus.
7. I understand and agree that when I take my pet outside, it will be on a leash, harness, or other tether and that I am
responsible for picking up the animals waste and appropriately disposing of it immediately. Additionally, I understand and
agree that my pet will wear proper identification at all times (unless the pet permanently resides in a cage) that includes
pet name and emergency contact phone number.
8. I understand and agree that if my pet is a caged pet, it must stay in its cage except when I am present and/or
attending to the pet.
9. I understand and agree that I must have a pet carrier in my possession and accessible in the event of an evacuation or
emergency.
10. I understand and agree that all cages and litter boxes must be cleaned of animal waste on a daily basis and disposed
of properly in the area designated by Residential Life.
11. I understand and agree that my room/suite/apartment will be subject to monthly health and safety inspections to
ensure it is sanitary and that the health and wellness of all occupants, including pets, are being maintained.
12. I understand and agree that I will assume the responsibility of regular hygiene for my pet (e.g., bathing, grooming)
and will use the proper treatments to prevent pests of any kind (e.g., fleas, ticks). I understand and agree that I may not
bathe or groom my pet in my residence hall room/suite/apartment or on university property and may not use university
laundry facilities for washing my pet’s bedding, toys or clothing.
13. I understand and agree that I am solely responsible for the actions of my pet, including any and all damage or injury
to persons or property caused by my pet. I understand that any cleaning and repair expenses incurred above and beyond
standard cleaning and repair to any university property will be billed in accordance with standard university policies and
procedures.
14. I understand and agree that my pet cannot be a threat to anyone living in the community. I understand that my pet
must not show aggression towards individuals or staff assisting in the community. Aggressive or repeatedly disruptive
pets will be required to be removed from university housing. Examples of disruptive behavior include, but are not limited
to, repeated barking or continuous noise, urinating/defecating on floors or walls, roaming the hallways, etc.
15. I understand and agree that my roommates, other students or guests cannot assume responsibility for the care of my
pet. I further understand and agree that my pet will not be left alone overnight in my room/suite/apartment, or in another
resident’s room/suite/apartment.
16. I understand and agree that I am responsible for arranging alternative care for my pet outside of the university (not
on campus) in case of an emergency or if I am required to remove the animal due to disruption, damage, etc.
17. I understand and agree that I will accept responsibility for carrying out all reasonable requests made by roommates -
including but not limited to the terms of our roommate agreement - and/or university staff regarding health, safety and
comfort of the community directly related to the presence of my pet in my residence hall.
18. I understand and agree that all complaints about my pet will be investigated by Residential Life, in conjunction with
the Dean of Students, Student Conduct and/or Campus Safety & Security if necessary, with possible sanctions imposed if
violations are found.
19. I understand and agree that the university may relocate me and my pet in residential housing in accordance with the
Room Terms Agreement and applicable Residential Life policies, procedures and rules.
20. I understand and agree that if I decide to no longer have my pet live with me or if the pet is no longer living that I will
notify Residential Life immediately. I understand and agree that if I want to bring a different pet to campus during the
same fiscal year, I will not be incurring additional $250 pet deposit, however I must go through the approval process prior
to making the change and bringing the different animal to live with me.
21. I understand and agree that the non-refundable fee of $250 has been paid to the university in advance of my pet’s
arrival to cover the cost of cleaning and repairing the room to a state where there is no evidence of the pet’s presence in
the room once the pet leaves housing. I understand and agree that, notwithstanding the fee, I will be responsible for all
additional costs and expenses that result from damage caused by my pet. I understand that my $250 deposit is annual
and valid for the entire fiscal year (July 1st through June 30th) and not transferable to the following year. Should I choose
to bring my pet the following year, I understand that I will need to pay the $250 deposit for that year.
22. I understand that I am required to attend a Residential Life-sponsored pet-friendly responsibility workshop after I
move in and that I am responsible for meeting the expectations outlined in the Pet-Friendly Guide.
23. I understand and agree that if I violate the terms of this agreement I will be asked to remove my pet permanently
from residential housing within 24 hours of being notified; and if my pet is removed, I understand and agree that I am
still bound to my housing contract (Room Terms Agreement) and may be required to relocate from the Pet-Friendly
Community. I understand and agree that if I fail or refuse to remove my pet from campus, the university will do so on my
behalf.
24. I understand and agree that if there is a medical emergency, including but not limiting death of the pet while on
campus, I must contact a local veterinarian or animal hospital. Further, I understand and agree that any deceased animal
is not allowed to be buried anywhere on campus grounds and must be immediately removed from campus.
