J. Wilkes Berry Scholarship Application Form
GENERAL INFORMATION: Applicants will not be awarded in consecutive years.
♦ One $250 scholarship will be awarded to a permanent full-time or permanent part-time TWU staff member enrolled in
undergraduate or graduate classes at TWU during the 2019 spring semester
♦ Applicants do not have to be member of ASSET to apply for this scholarship
♦ Applicants must submit a complete application packet by the posted deadline to be considered for the scholarship. An
application packet consists of a minimum of (3) Scholarship Recommendation Forms, an essay of 250-300 words to the
scholarship committee stating career objectives and a completed copy of this Scholarship Application Form.
♦ Application packets incomplete by or received after the deadline will not be considered for the scholarship
♦ The applicant is responsible for insuring that the Scholarship Committee Chair receives all materials by the deadline
stated below

Applicant: Please answer all questions on this form. This form, your career objectives essay and three (3)
recommendation forms must be returned to the ASSET Scholarship Committee Chair
by 5:00 PM on November 23, 2018
