ASSET Scholarship Application Form
GENERAL INFORMATION: Applicants will not be awarded in consecutive years.
♦ One $250 scholarship will be awarded to a permanent full-time or permanent part-time TWU staff member enrolled in
undergraduate or graduate classes at TWU during the 2019 fall semester
♦ Applicants do not have to be a member of ASSET to apply for this scholarship
♦ Applicants must submit a complete application packet by the posted deadline to be considered for the scholarship. An
application packet consists of (3) Scholarship Recommendation Forms, a letter to the scholarship committee stating
career objectives and a completed copy of this Scholarship Application Form.
♦ Application packets incomplete by or received after the deadline will not be considered for the scholarship
♦ The applicant is responsible for insuring that the Scholarship Committee Chair receives all materials by the deadline

Applicant: Please answer all questions on this form. This form, your career objectives letter and three (3)
recommendation forms must be returned to the ASSET President by 5:00 PM on May 31, 2019.
