SECTION A: STUDENT INFORMATION
Your Free Application for Federal Student Aid (FAFSA) indicates you are an independent student because you answered
YES to the one of the following questions:
At any time since you turned 13, were both your parents deceased, were you in foster care, or were you a dependent or ward of the
court?
OR
As determined by a court in your state of legal residence, are you or were you in legal guardianship?

If more than one category below applies to you, you only need to provide documentation for one category.

SECTION C: DEPENDENCY QUESTIONS
At any time since I was age 13…
Acceptable documentation:
Both my parents were deceased.
Copies of both parent’s proof of death.
(Such as death
certificates.)
I am or was appointed to a legal guardian by a
court in my state of legal residence.
Official court documentation indicating the name of the
guardian and the date guardianship was granted.
I am or was in foster care.
Official documentation of your participation in the foster care
system.

I am or was a ward of the court.
Official documentation from a court indicating you were
placed in the care of the court. (Note: Incarceration is not
considered ward of the court for the purposes of student financial aid.)
I made an error on my FAFSA, none of the above situations apply to me.
If none of these categories applies to your situation, please go to www.fafsa.gov. Correct the appropriate
question(s) in the Dependency Status section and include the requested information in the Parent
Demographic section. Your parent will need a FSA Client ID to complete this information.
Once MC has received your updated FAFSA information, submit this form and the additional Dependent forms
requested by the OSFA.
I certify that all the information contained on this form is complete and correct.
NOTE: If you purposely give false or
misleading information, you may be imprisoned and/or fined up to $25,000. I understand that this form must be signed
and submitted with all other requested documents before my financial aid will be processed. Electronic signatures are not
accepted.
