Selective Service Status Confirmation
& Information Form

Federal regulations require men aged 18-25 who were born on or after January 1, 1960, to register with the Selective
Service System within 30 days of their 18th birthday. This includes all men who are US citizens and non-citizens, who
reside permanently in the U.S. Men with Resident Alien Cards (Green Cards), must register. Men living in the U.S.
without INS documentation (undocumented aliens) must also register.
A Lawful Non-immigrant man who is in the United States temporarily (diplomatic corps, trade mission personnel, a
man holding valid visitor or student visas, etc.) is not required to register as long as he maintains that status.
Men are not required to register after reaching age 26. A man who is a non-citizen who first enters the U.S. as a
permanent resident after age 26 is not required to register because he is over the age limit.
Initial the statement below that best identifies your status and
submit in person documentation as directed:
1). I am registered with the SSS.
Proof of your Selective Service Registration to the MC Financial Aid Office. This information may be obtained by visiting
the SSS website at www.sss.gov,
or by calling 1-847-688-6888.
2). Currently under the age of 18
If you have not reached your 18th birthday, you must submit a copy of your birth certificate or another
government issued form of identification to confirm your date of birth (i.e. state ID card, driver’s license, and
passport). ________
3). I was born a female
A birth certificate. Only persons assigned the sex of male at birth are required to register. If a student’s gender identity is
now female but she was assigned the sex of male at birth, the student must register for SSS regardless of subsequent
sexual reassignment surgery or any other gender transition process. If a student’s gender identity is now male but he was
assigned the sex of female at birth, the student is not required to register for SSS, regardless of subsequent sexual
reassignment surgery or any other gender transition process.
According to information from the U.S. Department of Education, your registration with the Selective Service System
(SSS) could not be confirmed. As a result, you are not eligible to receive federal financial aid. You may appeal this
status by completing and submitting this form with all required documents.

4). A male born prior to 1960
Official government issued document showing date of birth such as state ID card, driver’s license, passport, birth
certificate.
5). Non-U.S. male who came into this country for the first time after his 26
th
birthday
Date of entry stamp in passport, I-94 with date of entry stamp on it, or a letter from the U.S. Citizenship and Immigration
Services (USCIS) indicating the date the man entered the United States. If the man entered the U.S. illegally after his
26
th
birthday, he must provide proof that he was not living in the U.S. from age 18 through 25. Resident Alien Card (Green
Card) is not valid as proof of the date of entry to the United States.
6). Non-U.S. male on a valid non-immigrant visa
For example, if the man entered the United States as an F-1 student visa and remained in that status until his 26
th
birthday,
he would need to provide documentation indicating that he was admitted on an F-1 visa and attended school full-time as
required. Acceptable documentation for this situation include a copy of his I-20 form or a letter from the school he
attended indicating his full-time attendance as a non-immigrant alien. The same thing applies for all non-immigrant
statuses.
7). A veteran
DD-214 or current fulltime active duty orders, military ID card. For men over age 26 who failed to register, active duty U.S.
military service is compelling evidence that their failure to register was not knowing and willful. These men need to
provide a copy of their DD Form 214, 'Certificate of Release or Discharge from Active Duty,' showing military service in the
armed forces - other than the reserve forces, the Delayed Entry pool, and the National Guard.
8). I Failed to register with Selective Service
You need to visit the Office of Student Financial Aid regarding the selective service appeal process.
