Foster Liability Waiver
I, _____________________________________________,
hereby agree to hold the Providence Animal Rescue League, its
Trustees, Executive Director, employees, agents, and assigns
harmless from any and all liability on account of any personal
injury, which may I may sustain while fostering an animal or
animals for the Providence Animal Rescue League, whether
onsite or offsite, from the Providence Animal Rescue League
property.
I acknowledge that I participate freely and of my own volition
and understand the risk associated with my participation, as it
may involve animals both known and unknown to me.
This release extends to all personal injuries sustained by me. I
assume responsibility for my own conduct and behavior while
participating in this foster care program, whether or not I am
on Providence Animal Rescue League property or offsite.
This release shall extend to and is intended to include a
personal release of the Executive Director, individually, as well
as the employees and assigns of the Providence Animal Rescue
League for any injuries I may sustain. I also hold the
Providence Animal Rescue League, its Trustees, Executive
Director, employees, agents and assigns harmless from any
damage to my personal belongings or apartment/home caused
by the animal(s) I am fostering.
