I agree that the security camera system was purchased and installed on the property after July 1,
2020
I agree that the APPLICANT is the property owner, or if not, has received consent of the property
owner to install the camera system.
I agree that the security camera system is installed on the exterior of a building.
I agree that the amount of the rebate does not exceed the cost of the purchase of the camera
system and the rebate amount is an actual cost incurred by the APPLICANT and is supported by detailed
documentation (i.e., receipts).
I will not use the security camera for any unlawful or harassing purposes or to intentionally record
specific individuals conducting lawful activity. I further agree to comply with all applicable building and
electrical code requirements.
I agree to indemnify, defend, and hold harmless the City of Kankakee government and its
authorized representatives from any and all claims, actions, losses, damages, and/or liability arising out
of this agreement except where indemnification is prohibited by law.
I agree that, the rebate Administrator, or designee, may make at least one on-site visit to the
address of the APPLICANT to verify camera installation.
I understand that false or misleading information in my application may result in denial of
assistance.
