When to use this form
Use this form only when you wish to apply for a tuition fees loan for a
degree programme at a Dutch university or university of professional
education (hbo) for the first time. Do you have Dutch nationality? Apply
for a loan for tuition fees via Mijn DUO at duo.nl.
Are you a student in secondary vocational education (mbo) or in general
secondary vocational education for adults (vavo)? If so, please use the
form Application for a contribution to course fees for students from EU/EEA countries
instead of this form. You can download it from duo.nl.
When to apply?
The application must be submied during the college year of the study
programme for which the tuition fees loan is requested.
An academic year runs from 1 September to 31 August.

Citizen service number
You will automatically obtain a citizen service number when you come to live
in the Netherlands and when you register with the municipal authorities.
Note: we cannot process your application form if you do not state your
citizen service number.

First name and surname
State your first name and surname as listed in your record within the
municipality.

Nationality
Send a copy of your passport or your EU permit.

Postal address
State your postal address here if you do not wish to receive mail from
DUO at your home address.

Application tuition fees loan
You can apply for a loan to pay your tuition fees, the tuition fees loan.
You receive your loan in monthly instalments. You are required to
repay the tuition fees loan aer you finish your degree programme.

Do not borrow more than you need to cover your tuition fees.
Dit formulier
Gebruik dit formulier uitsluitend als u voor het eerst collegegeldkrediet
aanvraagt voor een opleiding aan een hogeschool of universiteit
in Nederland. Ontvangt u al studiefinanciering? Vraag dan het
collegegeldkrediet aan via Mijn DUO op duo.nl.
Bent u student in het middelbaar beroepsonderwijs (mbo) of in het
volwassenenonderwijs (vavo), dan moet u het formulier Aanvraag
vergoeding lesgeld voor studenten uit EU/EER-landen gebruiken in plaats van dit
aanvraagformulier.
Wanneer aanvragen?
De aanvraag moet ingediend worden binnen het studiejaar, geldend
voor de studie waarvoor collegegeldkrediet wordt aangevraagd. Een
studiejaar loopt van 1 september tot en met 31 augustus.

Burgerservicenummer
U krijgt automatisch een burgerservicenummer als u naar Nederland
komt en u inschrij bij de gemeente.
Let op: vult u geen burgerservicenummer in, dan kunnen wij uw
aanvraagformulier niet verwerken.
1.2
Voor- en achternaam
Vermeld uw voornaam en achternaam zoals deze bij de gemeente staan
geregistreerd.
1.4
Nationaliteit
Stuur een kopie mee van uw paspoort of van uw EU-vergunning.

Postadres
Als u de post van DUO niet op uw woonadres wilt ontvangen, kunt u hier
een postadres invullen.

Aanvraag collegegeldkrediet
U kunt een lening aanvragen voor het betalen van uw collegegeld, het
collegegeldkrediet. Deze lening wordt maandelijks uitbetaald. Het
collegegeldkrediet is een lening die u na afloop van uw studie altijd
terug moet betalen. Kijk op duo.nl voor meer informatie over het
collegegeldkrediet en de terugbetalingsregels. Leen niet meer dan u
nodig hee om uw collegegeld te betalen.
