Sometime after closing your home loan you requested information regarding protecting your
loan in the event of your death, illness or disability. We show no record that this was handled,
so I have been assigned to your case.

1. If you are married, both you and your spouse must participate during the phone
consultation.
2. You must be prepared to send me a photo of your driver’s license so that I can verify
your identity to the insurance company. (Normally I would request it at your in person
consultation.)
3. Any mortgage protection policy submitted must be done on the same day of your
phone consultation so that medical information does not change in the application
from the consultation.
4. You will be signing your mortgage protection application by DocuSign as sent to you
directly from the insurance carrier after I have entered the information into the
application. Therefore, you must have a valid email address so that you can open the
application and sign it.
