Procedure 54-5:
Withdrawing Medication from Single-Dose or Multiple-Dose Vials
Objective: Withdraw medication from single-dose and multiple-dose vials.
Equipment and Supplies: Disposable gloves; sharps container; soap; needle; syringe; alcohol
wipe; medication vial

NOTES
1. Check the medication as
instructed in the “three befores”
technique before beginning.
Compare the medication vial
(bottle) against the physician’s
order (Figure A).
2. Select the correct syringe and
needle based on the type of
medication and location for the
injection site.
3. Perform hand hygiene and apply
gloves.
4. Roll the medication vial between
your hands to mix any medication
that has settled on the bottom.
Do not shake as this will create air
bubbles.
5. Wipe the rubber stopper with an
alcohol wipe firmly in a circular
motion. Then set the vial on a
clean surface while you prepare
the syringe (Figure B).
6. Remove the protective cap from
the needle on the syringe. In
some circumstances, such as
in home health or if the needle
needs to be transported safely
to another location, it may need
to be recapped. In this case,
after removing the protective cap
from the needle on the syringe,
maintain the sterility of the inner
surface of the protective cap
because it will be needed to cover
the needle again after you have
filled the syringe. However, many
needles have automatic retraction
or sheathing devices and do not
need to be recapped. Follow the
manufacturer’s directions for the
type of needle you are using.

7. Withdraw the syringe plunger and
allow air to enter the syringe in
an amount equal to the amount
of medication to be withdrawn.
Because the vials are vacuum
sealed, this will allow for easier
withdrawal of fluid.
8. Turning the vial upside down at
eye level and using care not to
touch the rubber stopper, insert
the needle into the rubber stopper
and inject the air into the vial. Be
extremely cautious concerning
contamination as you enter the
multiple-dose bottle.
9. Keeping the upside-down vial
at eye level, slowly withdraw the
correct amount of fluid medication
(Figure C).
10. While the needle is still in the
vial, check to make sure that
the dosage is accurate. Any air
bubbles in the syringe will give
you an inaccurate dose, because
they take up the space needed
for medication. To remove air
bubbles, flick your fingers against
the side of the syringe until the air
bubbles go back into the tip of the
syringe (Figure D).
11. Remove the needle from the vial.
12. If you have accidentally withdrawn
too much fluid, discard the excess
fluid by shooting it into a sink or
waste receptacle. Never return
medications to the vial or bottle
from which they came.

13. Check the medication vial after
you have withdrawn the dosage
to make sure you are correct as
instructed in the “three befores”
for checking medications. Also,
check to see if the multiple-dose
vial should be refrigerated after
opening.
14. If your facility policy requires
wearing gloves during injections,
remove and dispose of the gloves
after the medication has been
administered. Perform hand
hygiene.
