The following documentation is required to correct owner/vehicle information when an incorrect Certificate of Title or Certificate of
Origin has been assigned and an Illinois title issued:
New Vehicle:
1. Application for Title Only and Application for Corrected Registration for each purchaser involved in the title switch.
2. Correct, properly assigned Certificate of Origin.
3. Incorrect Illinois Certificate of Title with applicable lien release(s).
4. $153 fee payable to Secretary of State ($150 for title correction, $3 for corrected registration).
5. Title Switch Affidavit completed in full (see 2nd page).
6. An amended tax form (RUT 25X, RUT 50X OR ST556X) must be provided.
Used Vehicle:
1. Application for Title Only and Application for Corrected Registration for each purchaser involved in the title switch.
2. Correct, properly assigned Certificate of Title. If you do not have the correct properly assigned Certificate of Title, please
submit your incorrect Title with the Law Enforcement Certification attached.
3. $153 fee payable to Secretary of State ($150 for title only, $3 for corrected registration).
4. Title Switch Affidavit completed in full (see 2nd page).
5. An amended tax form (RUT 25X, RUT 50X OR ST556X) must be provided.

Due to new procedure that have been implemented, you must also include an Amended RUT-50 if you purchased this vehicle from an
individual. Dealerships must supply an Amended ST-556 with their transaction.
