This form is used to notify the Cascadia International Programs (IP) Office of your plans to transfer
your immigration record to another school. You must submit proof of acceptance to the new school
before your SEVIS record can be transferred.
I understand that in order to maintain status during transfer:
I must begin classes at the transfer school the next available term.
I must begin studies at my new school within 5 months of completion of my studies at Cascadia or the
end of OPT. Otherwise I must depart the U.S.
I may only choose one school to transfer my immigration record to; I will wait to request a transfer until I
have been accepted to the new school and decided to attend that school.
Once my SEVIS record transfers out, Cascadia cannot make changes or cancel the transfer.
I may transfer my immigration record to a new school at the end of my current academic quarter, or
within 60 days of completion of my studies or my OPT end date.
Any OPT or on-campus employment will be cancelled on the date of my SEVIS record transfer.
International travel restrictions apply during transfer— I will talk with an IP advisor before travelling.
My new school can only issue me a new I-20 after the transfer release date.
I understand the requirements above. If I change my decision, I must inform Cascadia College before my
SEVIS record is released to my new school; after this day my SEVIS record cannot be changed.
