2020-2021 IDENTITY VERIFICATION AND STATEMENT OF EDUCATIONAL PURPOSE
In order to process your 2020-2021 financial aid application, the Office of Financial Aid at Henry Ford College must verify your
identity as well as witness your signing of the Statement of Educational Purpose below.

Identity Verification
You must verify your identity by submitting a copy of an unexpired, valid government-issued photo identification (ID), such as, but
not limited to, a driver’s license, other state-issued ID, or passport. HFC will maintain a copy of the photo ID you present to our
office. This document, along with a copy of your photo ID may uploaded via your FA Checklist in the HFC student portal or by
mailing to the Office of Financial Aid.
