On your 2020-2021 Free Application for Federal Student Aid (FAFSA), you answered “Yes” to either question 53, which asks, “As
determined by a court in your state of legal residence, are you or were you an emancipated minor?” or to question 54, which
asks, “Does someone other than your parent or stepparent have legal guardianship of you, as determined by a court in your
state of legal residence?” Before the Office of Student Financial Aid can determine your eligibility for financial aid, you must
provide documentation of your status.
Instructions
Please check the box below beside the description that applies to your situation and attach the requested documentation.
Completed forms may be submitted by dropping them in a drop box on the HFC campus or by mail or fax to the address and fax
number listed above.
I am or I was an emancipated minor. Check this box only if:
(A) You can provide a copy of a court’s decision that as of today you are an emancipated minor, or
(B) You can provide a copy of a court’s decision that you were an emancipated minor immediately before you reached the age of
being an adult in your state.
Note: The court must be located in your state of legal residence at the time the court’s decision was issued.
Documentation: Attach a copy of a court’s decision of your emancipated minor status.
I am or I was in legal guardianship. Check this box only if:
(A) You can provide a copy of a court’s decision that you are in legal guardianship, or
(B) You can provide a copy of a court’s decision that you were in legal guardianship until you reached the age of being an adult in
your state.
Note: The court must be located in your state of legal residence at the time the court’s decision was issued.
Documentation: If (A) above applies to you, attach a copy of the Letters of Guardianship. If (B) above applies to you, attach a copy of
the court’s Order of Discharge from Guardianship (proof of prior guardianship).
I made an error on my FAFSA.
I am still a minor and the court decision is no longer in effect.
The court decision was not in effect at the time I became an adult.
Requirement: Correct the information on your FAFSA by 1) correcting your answer to question #53 and/or #54 and 2) updating your
FAFSA to include parent information. In addition, your parent must electronically sign the FAFSA.
By signing this document, I certify that all the information provided is complete and correct. I understand
that purposely giving false or misleading information may result in being reported to the Department of
Education.
