2020-2021 TIP Eligibility Form

I have been notified by the State of Michigan that I am eligible to participate in the Tuition Incentive Program
(TIP). I am requesting that Henry Ford College review my information and award any TIP funds I may be
eligible to receive. I have completed the necessary paperwork and have submitted it to the State of Michigan.
In order for Henry Ford College to determine a student’s eligibility, all TIP students must select HFC as their
college of choice with the State of Michigan. This is done via the Student Portal of the Michigan Student
Scholarship Grant (MISSG) website at www.michigan.gov/ssg.
Please check the appropriate box and return to the Office of Financial Aid.
I am eligible to receive Phase I TIP
To be eligible for Phase I, you must:
have graduated from high school (or completed your GED) before you turned 20 years of age
be a U.S. citizen, permanent resident, or approved refugee.
enroll in a program on at least a half-time basis (6 credits) within four years of graduation from high
school or GED completion.
submit the 2020-2021 FAFSA and include the HFC School Code (002270)
have your official high school transcript on file with HFC Office of Admissions
I am eligible for Phase II TIP
To be eligible for Phase II, you must:
have 56 transferable semester or 84 transferable term credits.
have earned an associate degree or certificate (minimum of one year)
submit the 2020-2021 FAFSA and include the HFC School Code (002270)
have your official high school transcript on file with HFC Office of Admissions
Once Phase II TIP benefits have been initiated, a student cannot use any remaining Phase I benefits.
I am a Guest Student at HFC
To be eligible for TIP funding as a guest student, you must:
submit this form.
have your official high school on file with HFC Office of Admissions
submit the 2020-2021 FAFSA and include the HFC School Code (002270)
complete the TIP Guest Student Verification form located on the State of Michigan website:
www.michigan.gov/ssg

that purposely giving false or misleading information may result in being reported to the Department of
Education.
