SECTION A: STUDENT INFORMATION
Your 2019-2020 Free Application for Federal Student Aid (FAFSA) indicates you are an independent student;
however, you were processed as a dependent student in the previous award year. Please review each
question below and select YES or NO and follow the appropriate instructions.
• If you answer YES to any question below, submit this form along with the requested documentation in addition to any other forms requested by the
Office of Student Financial Aid (OSFA).
• If you answer NO to
every question below, you must go to www.fafsa.gov and correct your FAFSA. You will need to correct your answers in the
Dependency Status section. You will also need to complete the Parent Demographics section. (Your parent will need FSA Client ID.)
• Once you receive notification that your FAFSA has been received by MC, submit this form and the additional forms requested by the OSFA.

SECTION C: DEPENDENCY QUESTIONS
At the time you completed your FAFSA were you married?
If yes, attach a copy of your marriage
certificate.
At the beginning of the 2019-2020 school year, will you be working on a master’s
degree or doctorate program (such as an MA, MBA, MD, JD, PhD, EdD, graduate
certificate, etc.)?
If yes, attach a copy of your
undergraduate degree.
Are you currently serving on active duty in the U.S. Armed Forces for purposes other
than training?
If yes, attach a copy of your current
military assignment.
Are you a veteran of the U.S. Armed Forces?
If yes, attach a copy of your DD214.
Do you have children who receive more than half of their
support from you between July 1, 2019 and June 30, 2020?
If yes, for each child provide: name,
age, and proof of support.
Do you have dependents (other than your children or spouse) who live with you and
who receive more than half of their support from you, now through June 30, 2020?
If yes, for each dependent provide:
name, age, relationship to you, and
proof of support.
At any time since the age of 13, were both your parents deceased, were you in foster
care, or were you a dependent or ward of the court?
If yes, attach official documentation of
your situation.
Are you or were you an emancipated minor as determined by a court in your state of
legal residence?
If yes, attach court documented
emancipation papers.
Are you or were you in legal guardianship as determined by a court in your state of
legal residence?
If yes, attach court documented legal
guardianship papers.
At any time on or after July 1, 2018, did your high school or school district
liaison determine that you were an unaccompanied youth who was homeless?
If yes, attach documentation from your
high school or school district homeless
liaison.
At any time on or after July 1, 2018, did the director of an emergency shelter or
transitional housing program funded by the U.S. Department of Urban Development
determine that you were an unaccompanied youth who was homeless?
If yes, attach documentation from the
director of the emergency shelter or
transitional housing program.
At any time on or after July 1, 2018, did the director of a runaway or homeless youth
basic center or transitional living program determine that you were an unaccompanied
youth who was homeless or was self-supporting and at risk of being homeless?
If yes, attach documentation from the
director of the runaway or homeless
youth basic center or transitional living
program.
