I understand that financial aid is subject to adjustment or cancellation at any time if there is
a change in my household’s circumstances, if there is a change in enrollment status, or if
additional information warrants such an adjustment.
I understand that I must maintain satisfactory academic progress and must be in good
standing in the course of study I am pursuing, according to the standards and practices at
Montgomery College.
If requested, I agree to submit additional documentation in response to any request from
the Office of Student Financial Aid.
I certify that all statements and information provided on my application are true and correct
to the best of my knowledge, and understand I may be fined, sentenced to jail, or both, for
purposely giving false or misleading information on any form used to determine my eligibility
for financial aid.
I understand if this form or any other documents needed to process my application are
incomplete, they will not be accepted and/or will delay the processing of my financial aid.
I understand that in order to receive financial aid I must have graduated from high school or
have the equivalent of a high school diploma. If requested, I must provide one of the
following to validate my high school graduation or its equivalency:
To the Admissions and Records office:
• Final official High School Transcript
• Copy of my General Educational Development test results (GED)
• Valid Homeschool Completion Credential
• Foreign Education Evaluation, diploma, or leaving certificate
• State Authorized High School Diploma Equivalent
SECTION D: ADDITIONAL INFORMATION
If you feel that you, (or
your household), have unusual circumstances that warrant special consideration, please meet with a financial aid
counselor or specialist. Our office will work with you to determine if additional documents are needed that could affect your financial aid award.
All information you submit to the OSFA is secure and subject to the Family Education Rights and Privacy Act (FERPA).

If you want someone other than yourself to have access to information in your student file, you must be present with him or her or have written
authorization on file. This authorization form is available in the OSFA.

Once you have completed this, and all other requested forms, please return them together to the Office of Student Financial Aid.
These forms can be submitted in person or by U.S. mail. DO NOT fax or e-mail your forms.
Incomplete forms and/or inconsistent information will delay the processing of your financial aid.
Allow (5) business days for forms to update on your MyMC account.
