ALL NIGHT GRADUATION PARTY 2021
WAIVER AND RELEASE FORM
I am the parent/guardian of _____________________________________________ (Graduating Senior)
and give permission for my Graduating Senior to attend the All Night Graduation Party (“Event”) on June
25-June 26, 2021 from 11:00 pm - 4:00 am at Monroe-Woodbury High School located at 155 Dunderberg
Rd, Central Valley, NY 10917.
Both I and the Graduating Senior agree that we will not hold liable whatsoever, any member of the
Monroe-Woodbury HS PTA and/or Graduation Party Committee, Monroe-Woodbury Central School
District, or any of its representatives, for any injury incurred while in attendance, or while participating in
any event at the Monroe-Woodbury High School All Night Graduation Party.

Th
e
Graduate
will
NOT be
permitted
to
leave
the
All
Night
Graduation
Party
unless
he/she
becomes ill,
in which case the parent/guardian is required to pick up student
from event
.
If
the
Graduate
leaves
early
for
any
other
reason
a
previous
written
arrangement
must
be approved by
administration prior
to the event.
YOU WILL BE CONTACTED IF YOUR CHILD WISHES TO
LEAVE AND
THERE IS NO
WRITTEN
ARRANGEMENT ON
FILE.
●
The
Graduate
will
be
respectful
of
volunteers
and
classmates
while
in
attendance,
and
unacceptable behavior
may result
in dismissal,
in which case the parent/guardian will
be notified.
●
This
completed
waiver/permission
slip
MUST
be received by April
23,
2021
to be registered for
attendance at
the All
Night
Graduation Party.
●
A waiver
MUST
be signed regardless of
whether
the Graduate attends the Event.
●
Waivers
can
be
dropped
off
in
the
main
office
(labeled
ANGP)
●
The
Graduate
MUST
provide Photo ID
upon entering the event.
●
No
Bags
Allowed,
no
outside
food
or
beverages
and
no
guests
allowed.
●
The
Graduate’s
picture
or
likeness
may
be
taken
during
the
event
and
any
pictures
taken
during
the event may be published, without any other personal identifiers, in any form, on the All Night
Graduation
Party
site
and/or
social
media
sites.
