UTM OSAP REINSTATEMENT FOR JANUARY 2021
Complete this form if you have dropped below OSAP’s minimum required course load (1.5 credits, or 1.0 credit
for students with a documented permanent disability on their OSAP file) in the Fall term and you plan on
returning to full‐time studies in the Winter term. Completion of this form will ensure that your Winter term
is included in your 202021 OSAP assessment.
When you drop below the minimum required course load, OSAP recalculates your funding entitlement only up
to the date of your drop. This typically results in you being overpaid OSAP funding in the Fall term, as you no
longer qualify for the full amount of funding you originally received for the entire term. Overpayments of
OSAP may lead to other penalties, such as grant to loan conversions or restrictions from funding. Completion
of this form may reduce that overpayment and the impact of those penalties. This form will also reinstate
interest‐free status to your OSAP loans for the Winter term.
Important Notes:
1. Students must maintain the minimum required course load, as outlined above, in each term, in
order to maintain their eligibility for OSAP. They must also pass a total of 3.0 credits over the Fall‐
Winter Session (2.0 credits for students with a documented permanent disability).
2. Multiple program switches, failed courses, drops or withdrawals are also considered to be a lack
of progress and academic direction.
3. Failure to maintain satisfactory academic progress in two consecutive years of study will result
in ineligibility for further OSAP assistance.
Instructions for Student:
1. Complete all sections of these forms. Ensure that the forms are signed. Applications can be
electronically signed. The forms have been made fillable for your convenience.
2. Upload completed forms to the AskRegistrar Service. After submission of these forms, it will
take our office approximately 4 weeks to process. You should check your file on the OSAP
website (ontario.ca/osap) for your updated assessment once it has been processed.
3. Students are encouraged to upload this form and any supporting documentation (if applicable) as
soon as possible, and no later than March 13, 2021. This will allow any additional OSAP funding,
that you may be eligible for, to be issued promptly and reinstate interest‐free status on your
previous loans. Late or incomplete forms will not be considered.

Purpose
Use this form if both of the following statements apply to you:
You are on academic probation or are returning to school after a period of academic probation.
You have submitted a 2020-21 OSAP Application for Full-Time Students.
Your eligibility for OSAP funding will be on hold until your completed form is received and reviewed by your
financial aid office or the ministry.
Required documents
A copy of your transcripts for your academic probation period may be required. You do not have to
purchase official transcripts.
See ‘Section B: Required documents’ to determine if you need to provide a copy of your transcripts.
Review of academic probation
You may be eligible to have your academic probation removed. Contact your financial aid office if any
of the following impacted your ability to maintain satisfactory academic progress:
a new or pre-existing medical condition;
a disability-related issue; or
a family crisis.
How to submit this form
The quickest way to submit your form is to upload it to your OSAP account. Log into the OSAP website
and go to your application to use the “Print or upload documents” button. Or, you can submit a paper copy
as follows:

Deadline
This form and all required documents must be received by your financial aid office or the ministry no later
than 40 days before the end of your 2020-21 study period.
