Housing Contract Release Request
Please indicate your reason for a contract release request below:
____ Change in financial status. Please provide documentation of the change in financial status. This may include
updated FAFSA information, tax documents, etc. Credit card statements and bank account statements are not
sufficient documentation.
____ Medical or psychological needs that cannot be accommodated within the residence hall living

Medical documentation will remain confidential and is maintained by the Health and Wellness Center, who will
make a recommendation to the Office of Residence Life regarding your request.
I have fully read and understand the terms of the above contract release request. I acknowledge that all of the
information provided by me is true and correct. If the contract release is granted, I agree that I am relinquishing
my current room assignment, will move out within the reasonable time frame established by the Office of
Residence Life (typically two business days), and will need to reapply for housing if I later choose to return. I
understand that my housing and dining charges will be refunded on prorated basis, in accordance with the
College’s refund policy. I will provide my local address and current cell phone number to the College. I understand
that I may be subject to contract cancellation fees as outlined in the housing contract. Further, I understand
providing false information at any point in this process will result in disciplinary action.

Housing contracts are legally binding agreements between the student and Lake Forest College. Students may
request to be released from their housing contract due to a change in financial situation since signing the contract
or due to a medical or psychological condition which cannot be accommodated on campus. The contract release
request is a request; it is not a guarantee that the request will be granted, nor is it a waiver of related cancellation
fees. If your request is denied, you will remain responsible for all housing and dining plan fees pursuant to the
housing contract. Your request and documentation will be reviewed by a committee composed of representatives
from the Office of Residence Life, the Office of Admissions, the Business Office, and Financial Aid. Requests received
without documentation will not be reviewed. You will receive a response to your request within ten (10) business
days of submission of the form.
