I agree that loan documents including but not limited to, subject property evaluations, initial loan
disclosures and/or re-disclosures pertaining to the home loan transaction for the subject property

will be provided or made available either on paper or electronically, at the discretion of Paramount
Residential Mortgage Group, Inc. If you consent, your loan documents and any other customer
notices, including but not limited to: all privacy, truth in lending, electronic funds transfer notices
and home loan related documents will be sent electronically. You may withdraw your request to have
them sent to you in electronic form at any time by giving us five (5) working days advanced notice
in
writing. That notice may be sent by first-class mail to us at 1265 Corona Point Circle, Ste 301, Corona
CA 92879, or to your loan originator.
During the loan process, if you change your e-mail address, you must notify us of your new address
within five (5) working days by mail or secure email in order to receive notification. If Paramount
Residential Mortgage Group, Inc. receives a notice that your email address is unreachable or
undeliverable, your account will be switched to receive paper documents, disclosures, other
notifications, etc.

etc. via electronic means rather than postal mail or Federal Express each time such document would
otherwise be sent to me by postal mail or Federal Express. Please continue to do so until I withdraw this
