If I pay for a TEAS exam which exceeds the maximum three (3) test limit, then I will forfeit the fees paid for that exam and
I will not be permitted to take the test a 4th time in this period.
Registration fees are non-refundable and cannot be applied to the next calendar year.

in Kee Hall. Reserve early as seats are LIMITED! Exams can last up to four (4) hours, so please plan accordingly. ONLY ONE test date allowed
per form.

Follow these steps to pay and reserve your TEAS Test.

"Create an Account" with ATI Testing at
This mandatory account login will allow you to take your test, retrieve your results, and create a "focused review"
after testing. DO NOT attempt to register and pay for your test through the ATI website. Pay at the Cashier's office in
Kee Hall.
Take this form along with your cash, check, money order, or credit card to the Cashier's Office in Kee Hall AT LEAST 24
HOURS BEFORE YOUR TEST DATE. Make checks payable to North Central State College.
Prepare for the exam by using the ATI TEAS Study Guide (at Bromfield Library or Campus Bookstore), use the Tutoring
Center, or order online study materials directly from ATI at https://www.atitesting.com/teas-prep
Please bring a form of identification with you on the test day to Kee Hall, Room 138. Suggested forms of identification
are a driver's license, social security card, high school or college I.D. Please arrive at least 15 minutes prior to the test
start time.
If you cannot attend on the date indicated, you must notify the Student Success Center at
419-755-4764 at least 7 DAYS in advance of your scheduled test date to reschedule; otherwise, another
non-refundable fee may apply.
The following changes for the TEAS went into effect on August 26, 2013:
* MATH0074/MATH0084 (or higher level MATH or STAT) and CHEM1010 (high school chemistry) are pre-requisites to the
TEAS.
