I hereby certify that these students are eligible according to Sections 400, 430, and 1102 of the
Constitution and Contest Rules of the University Interscholastic League, and are eligible to
compete in the UIL music event named above.
(1) Attach to this form a roster of students, in alphabetical order by last name, who are eligible to compete in
the UIL music event named above.
(2) The roster may be amended the day of the contest to reflect the names of the students who are actually
eligible to participate. The amended list must be on file with the region executive secretary or designee
prior to the scheduled performance time for the organization whose name appears on this form. The
accuracy of the revised list remains the responsibility of the Principal or Superintendent whose signature
appears below.
(3) The following eligibility rules apply to the filing of this form.
