Facility Prior History Request
Records relating to child abuse and neglect investigations are closed and confidential under the Sunshine Law §§ 610.021(14) and
210.150 RSMo, except for very limited circumstances. Section 210.150.2(10) RSMo permits the Division to disclose details limi ted
to the nature and disposition of substantiated child abuse or neglect findings to “[a]ny person who inquires about a child abuse or
neglect report involving a specific child-care facility, child-placing agency, residential-care facility, public and private elementary
schools, public and private secondary schools, juvenile court or other state agency."

information, so consider indicating possible variations of facility name and addres s in your request (ex. 1234 W. Main St.,
1234 West Main Street, 1234 W. Main Street, 1234 West Main St.).
