Essay
Essay One (required): Write a letter stating your need for this scholarship, plans for your education, and your future goals and
plans beyond college (please limit response to 750 words or less and font must be 10 pt. or greater).
Essay Two (optional): Write a letter explaining any personal or family circumstances affecting your need for financial
assistance.
Financial Information
Please include a copy of your FAFSA Results (Student Aid Report) and/or college funding estimator worksheet (found at
educationquest.org
). In addition, please include a copy of the first two pages of your parent or guardians Federal Form 1040 or
Federal Form 1040A. Please also include Federal Form 1040EZ, if applicable. Please note SSN can be blacked out.

The following items must arrive no later than March 1st in order for an application to be considered complete. Only one
envelope with the completed application will be considered per applicant. At the top of each page, please type, or neatly print
your initials. Any applications received after March 1st will not be considered.
