(PRINTED NAME)
Lamar State College - Port Arthur, it is my responsibility to immediately notify the Veterans’ Representative on this campus. I also
understand that early notification will help prevent overpayment of Educational Benefits and that if overpayment does occur, I may be
responsible for full repayment. I also understand that every course I enroll in per semester at Lamar State College Port Arthur must be
on my current active degree plan.
If I feel there were mitigating circumstances requiring my drop/withdrew, I may petition the Veterans Administration by completing
Form 21-4138, “Statement in Support of Claim.”

cumulative GPA, the student may continue on probation for one additional semester/summer term.

If a student on probation fails to achieve a 2.00 semester/summer term GPA at the end of the first probationary period, the student shall
be reported to the VARO as making unsatisfactory progress.
A student who fails to achieve a 2.00 cumulative GPA at the end of the second probationary period shall be reported to the VARO as
making unsatisfactory progress.
I have read and understand this policy. I have also received a copy of this policy.
