Undergraduate Graduation Date Change Form
Graduation date change forms will be processed only if received prior to the graduation application
deadline. If you would like to withdraw your application for graduation you may do so at any time by
completing the Request to Cancel Graduation Application found online at www.unr.edu/academic-central
1. Meet with your major advisor for the review and approval of your request.
2. This request will be subject for review and approval by the Graduation Coordinator.
3. You may use this form only once to have the $95 graduation fee applied to a future term. Any additional changes to your
graduation date will incur a $95 fee per submission.
4. This form is only accepted until the graduation application deadline - October 1st (Fall), March 1st (Spring) or June 1st
(Summer).
5. Submit the Undergraduate Graduation Date Change form in person on the second floor of Fitzgerald Student Services or mail
