SCHOLARSHIP AWARDS CHECKLIST
ü Tab through the form to fill in the boxes online. The required fields are:
• Prepared by - First & last name
• Department - The department represented by the preparer
• Contact Number - Your UWF 7 digit telephone #
• Date Submitted - The date the form is completed & e-mailed to the Foundation
• Student ID - The UWF student 9 digit ID # (who will be awarded the scholarship)
• Student's Last name, First name, Middle Initial Example - Smith, John A.
• Award year - Ex: if Fall 2012 then “2012” goes in the column header
• Award amount- Dollar amount goes in each respective column
• University and/or Foundation Account Number & Name
• Enrollment Requirement - Either FT, HT, TT, or None
• Chair/Dean/VP - The department's responsible party who approves or authorizes awards from the respective
University and/or Foundation project/fund/account
ü All awards require the Chair/Dean/VP name field to be completed. Endowment and scholarship agreement criteria
provide guidance on whom selects recipient or authorizes awards.
ü Once all required fields are completed, save the PDF to your computer. You will need to e-mail the completed form to
Foundation@uwf.edu for Foundation Accounts and mixed use funding sources. Also, the Chair/Dean/VP indicated in the
typed field on the form must be copied on the e-mail to the Foundation.
ü Request for all other funds (no Foundation $) should be sent directly to Georganne Major in Financial Aid at
gmajor@uwf.edu.
ü Awards submitted after classes begin might not be processed in Financial Aid by fee payment due date.
ü Any late fee charges assessed by the University due to late awards will not be waived.
