U.S. High School Information Form for Placement
De Anza can consider U.S. high school transcripts for placement into the math, English & ESL course sequences.
Follow these steps to become eligible for a high school transcript (HST) assessment:
1. Apply to De Anza and get your student ID number or CWID deanza.edu/apply-and-register
2. Complete this form and email it to daassessment@deanza.edu
General Processing Guidelines
• We can only assess U.S. high school transcript information.
• You are not eligible for this type of assessment if you have not completed 11
th
grade yet. You’ll want to complete
another type of assessment https://www.deanza.edu/assessment/guided/index.html
• We reserve the right to request a copy of your high school transcript for auditing purposes.
• We will email you within 3-5 business days when your results are posted!
