Please complete this application thoroughly and thoughtfully. When completed, you must print the application and attach your Personal
Statement with any documentation. Applications WILL NOT be accepted in the mail via United States Postal Service. Students must either
hand deliver their personal statement and application materials or email them to their advisor prior to the reinstatement interview
(depending on COVID-19 circumstances).
Applications must be approved by your Academic Advisor on OR before the following deadline:
• Current students deficient under Rule 38a: 2pm on Friday, January 7, 2022
• All Former Students Returning (FSR’s): 5pm on Thursday, December 23, 2021
• Students deficient under Rule 38b Fall 2021, will be recessed from WSU for one academic semester following deficiency.
• Students deficient under Rule 39 Fall 2021, will be dismissed from WSU for two consecutive academic semesters.

and thoughtful personal statement, it will not be possible to give your application full consideration.
