New Club/Organization Intent Form
Groups interested in forming a new club/organization must fill out this form prior to approval from
The Office of Campus life to receive temporary recognition. Temporary recognition does not
allow the club/organization eligibility for student government funding but does permit the use of facilities
and services.
If you are interested in obtaining Club Sport recognition, the Club Sports First Year application
(available on-line at www.stthomas.edu/clubs) must accompany this form
Purpose / Mission of club or organization - explain how your club/org will contribute to the
mission of the University of St. Thomas:
Name of advisor (must be a full-time faculty or staff member of UST, include phone number,
email address, and mail number):
This organization will have four (4) weeks from the signature date to draft a constitution for approval.
Please submit 10 copies of the constitution to Campus Life. Failure to complete this process will result
in this organization's temporary recognition to be voided.
