GENERAL INFORMATION
A Historic Military Vehicle (HMV) is a vehicle manufactured for
use in any country’s military forces and maintained to accurately
represent its military design and markings with no alterations. HMVs
manufactured in the US may be of any age. Imported HMVs must be at
least 25 years old. An HMV may be a truck, trailer or motorcycle of any
size and weight, but cannot be a tracked vehicle.
NOTE: a tracked vehicle runs on continuous tracks or a combination of
wheels and tracks.
Historic military vehicle registration allows operation to and from special
occasions, such as parades and display events, club activities,
exhibitions and tours. Operation is also allowed for occasional personal
use as well as testing, maintenance and storage purposes. Operation of
this vehicle is prohibited until the application is processed and plates
issued. Applications for HMV title and plates must be mailed to Madison.
Local DMV Customer Service Centers are unable to process these
applications, however temporary plates can be issued there while
application is being processed.
By state law, Historic Military Vehicle license plates CANNOT be
personalized.

RELEASE OF NON EXEMPT INFORMATION
Under Wisconsin open records law, the Wisconsin Department of
Transportation must provide information from its records to requesters.
If you do not want your name and address included in requests we
receive for ten or more records, you may ask the department to withhold
your name and address from those lists by checking the box:

HOW TO APPLY
1. Complete and sign application; print clearly (if not completing
online).
Enclose COLOR photos for engine & all 4 sides of exterior

2. If the vehicle is NOT titled in your name in Wisconsin, include a
completed title application (MV1 or MV11), appropriate title fees,
current title being transferred or original US Gov SF97 form for
vehicles purchased from a government sale and the other
application materials shown in step 1 above to the address below.
3. If the vehicle is titled in your name in Wisconsin, send your original
title with the application materials shown in step 1 above to:

The fee for non-expiring Historic Military Vehicle plates is $5.00.
