You must prove lawful presence (this may be done online at mass.gov/rmv) and then mail or fax completed application to
the address listed above within 180 days of the class graduation date, along with the following:
1. Copy of your Massachusetts Motorcycle Learners Permit.
2. Copy of your Motorcycle Safety Foundation Completion Card.
3. Copy of your Basic Rider Course Skill Evaluation Score Sheet and Knowledge Test.
To get or renew any driver's license, ID card, or learner's permit, you need documentation showing U.S. citizenship or
lawful presence as required by federal and state law. This includes adding a motorcycle endorsement to your license.
Refer to the backside of this application for instructions.
Please note that copies of documents will not be returned.
If your application is approved, a new Massachusetts Drivers License with your motorcycle endorsement will be mailed to
you within 10-15 days of receipt this application.

New Massachusetts Requirements (effective 3/26/18)
1. Go online to mass.gov/rmv and select “Access My Profile”. Then follow prompts to verify your identity and start
your transaction.
2. Choose the type of card you want:
Standard Massachusetts driver’s license card. A Standard license is not valid as a form of federal identification
and cannot be used to board a plane in the U.S. or enter certain federal buildings after October 2020.
OR
REAL ID Massachusetts driver’s license. A REAL ID is valid as a form of federal identification and can be
used to board a plane in the U.S. or enter certain federal buildings after October 2020.
3. Our website will help you decide which type of card is right for you, and what documents you’ll need.
If you choose a Standard Massachusetts driver’s license, you may be able to submit all required documents and
complete the process online.
Prove “Lawful Presence”
1. Go back to your Profile
2. Click “More” in the top box
3. Click “Prove Lawful Presence
4. Follow the prompts to prove “Lawful Presence” online.
If you choose a REAL ID Massachusetts driver’s license, you must:
1. Start your application online at mass.gov/rmv.
2. Print your application summary document.
3. Visit an RMV Service Center with your application summary document and required documentation.
Pay Motorcycle Endorsement
1. Access mass.gov/rmv and click on Online Service Center
2. Select ‘Access My Profile’ and follow the prompts
3. Click ‘More’ in the bottom box
4. Click ‘Pay License and Road Test Fees
5. Pay your fees
If you proved lawful presence and paid your motorcycle endorsement fee online fax all of your paperwork to 413-
455-3646 attention Gene Carabine, MREP Coordinator or email your documents to
Eugene.carabine@state.ma.us.
If you could not pay the motorcycle endorsement fee ONLINE then you must mail a check or money order for $15.00
endorsement fee, made payable to the MassDOT If your application is not approved, the $15.00 application fee will
be returned to you.
If you could not prove lawful presence online, you must visit an RMV Service Center with all of your paperwork.
