Declaration of parent/guardian
1. I confirm that I have arranged for the above mentioned minor to be accompanied
to the airport on departure and to be met at stop over point and on arrival by the
persons named. These persons will remain at the airport until the flight had departed
and/or be available at the airport at the scheduled time of arrival of the flight.
2. Should the minor not be met at stopover point or destination, I authorize the
carrier(s) to take whatever action they consider necessary to ensure the minor’s
safe custody including return of minor to the airport of departure, and I agree to
indemnify and reimburse the carrier(s) for the necessary and reasonable costs and
expenses incurred by them in taking such action.
3. I certify that the minor is in possession of all travel documents (passport, visa, health
certificate, etc.) required by applicable laws.
4. I the undersigned parent or guardian of the above mentioned minor agree to and
request the unaccompanied carriage of the minor named above and certify that the
information provided is accurate

Verklaring ouder/begeleider
1. Ik bevestig dat ik mij verzekerd heb van begeleiders voor bovengenoemde
minderjarige naar de luchthaven van vertrek, op een tussenstop en bij de luchthaven
van aankomst door opgegeven personen. Deze personen blijven op de betreende
luchthavens beschikbaar tot de vlucht is vertrokken/gearriveerd.
2. Mocht de minderjarige niet worden ontmoet bij de tussenstop of worden afgehaald
op de eindbestemming dan machtig ik de vervoerder tot elke actie noodzakelijk, om
verzekerd te zijn van de veiligheid van de minderjarige en te zorgen voor terugkeer
van de minderjarige naar de luchthaven van vertrek. Ik ga akkoord met vrijwaring van
en terugbetaling aan de vervoerder voor de noodzakelijke kosten gemaakt om deze
actie te ondernemen.
3. Ik verklaar dat de minderjarige in het bezit is van alle reisdocumenten (paspoort, visa,
gezondheidsverklaringen etc.) zoals deze vereist zijn door de van toepassing zijnde
wetten.
4. Ondergetekende stemt hierbij toe in vervoer van bovengenoemde minderjarige en
verklaart deze aanvraag naar waarheid te hebben ingevuld.
