As the trip leader for the trip listed on this form, I certify that this trip is purely of a noncommercial nature. No money
has been collected beyond the direct costs of the trip including expenses for food and transportation. I have not and
will not receive any remuneration (gain) for my efforts as the trip leader. I understand that the Forest Service may
check information I have provided, and the rangers may ask questions about my group. I understand that giving false
or misleading information to a federal officer is a violation of C.F.R 361.3(b)giving any false, fictitious or fraudulent
report of other information. This violation is punishable by up to six months in jail and/or a $5,000 fine. I also
understand that violating any of the noncommercial trip requirements by offering or selling any services or
merchandise is considered "illegal outfitting (C.F.R. 261.10(c))," and this violation is punishable by up to six months in
jail and/or a $5,000 fine.

The Snake River Fund is a nonprofit organization that supports the Jackson Ranger District river program and the
Snake River. The Snake River Fund is a voluntary donation program that takes the place of a mandatory fee
program. With support from river users like you, we will be able to keep the Snake River fee free. The Snake
River Fund shares event and river information with river users through their website at www.snakeriverfund.org
through an e-mail listserv. Please check the box to the left if you would like us to share your contact information
with the Snake River Fund.

**NOTICE!! We will reply within 5 days of receiving your application via e-mail or phone to confirm receipt. If you
do not hear from us within 5 days, then we have not received your application! If this happens, you need to
contact us as soon as possible via e-mail or phone. r4_b-t_snake_river_permits@fs.fed.us or 1-307
-739-5444. You
must apply 2 weeks in advance to give us time to process your application due to the large number of applications
we receive. Permits are issued by river rangers and the permit program is funded with limited funding through the
Snake River Fund. Time and funding are restraints on this program and that is why we request you apply two
weeks in advance. If you do not have two weeks advance to apply, it is in your best interest to contact us via e-
mail or phone to see if the resources are available to process a late request.
**A state AIS boat decal is required by the state of Wyoming to launch. To prevent the spread of Aquatic Invasive
Species (AIS), you must clean, inspect, and dry your boats and equipment prior to visiting the Snake River. For
