more of your classes for the current semester, and that you are registered for one or more classes
that have not yet started. In accordance with federal regulations, FAS is required to confirm your
intent to attend and complete the classes that have not yet started.
Please complete this form and return it to FAS to confirm your intent to attend and
complete the classes that you have not yet started for this semester. Failure to return this
form may result in your financial aid awards being adjusted. If you do not plan to attend and
complete your additional classes this semester, you must officially drop the class(es) on the web
or contact Records and Registration.
