EX PARTE APPLICATION FOR RESTORATION OF FORMER
NAME - AFTER ENTRY OF JUDGMENT
Purpose of Instructions
This packet is designed to help you have your former name restored, after you have had a
Judgment entered in a dissolution case.
What Forms are Required
“Ex Parte Application for Restoration of Former Name”, FL-395
You must file the “Ex Parte Application for Restoration of Former Name”, FL-395, along
with one copy at the Family Relations Courthouse. You will also need to include a copy
of your Judgment. You may file this document either in person or by mail. If you file by
mail, be sure to enclose a self-addressed envelope with enough postage to return the filed
copy back to you. The address is:

There is no fee to file this form.
In order to change your name, some places, such as the Department of Motor Vehicles or
the Social Security Office, may want to see a certified copy of this Order. There is a fee
for the certification of the Order and you may request that the Order be certified and pay
the fee at the time you submit the Order or after you receive the signed Order back from
