Please be aware that this appeal does not guarantee approval for an additional unsubsidized Direct
Loan. Parents should attempt to apply for Federal PLUS Loan first at www.studentloans.gov. If
approved, and your parent meets one of the following criteria listed below, you can request a review
by filling out the form and providing required third party documentation to support your request.

Your parents are separated and your custodial parent is incarcerated (attach documentation)
Your parent is not a U.S. citizen or eligible non-citizen (attach documentation for both parents)
Your parent’s whereabouts are unknown (provide statement below indicating the last time you have
had contact with BOTH parents):
