□ Your Direct Loans will be Awarded for Summer/Fall/ Spring and be accepted and divided in three
amounts .Please check which loans you would like:
o Subsidized and Unsubsidized
o Subsidized Only
□ All applicant requirements under the Financial Aid Tab-Eligibility, requested in LOLA at Louisiana
Delta Community College Self-Service, are verified and complete.
□ I have completed the online Loan Entrance Counseling and Master Promissory Note online at
www.studentloans.gov. (You will need your FSAID and password that you used to complete your
FAFSA).
