Ride Share and Delivery

Supplement

Plan Benefits:

• Telephone Consultation and Legal Research. Phone

consultations on an unlimited number of Ride Share and

Delivery issues with up to 2 hours of legal research concerning

Ride Share and Delivery issues.

• Calls, Letters, or Document Review. The Member may

choose to have 3 phone calls or letters made on its behalf or

documents reviewed (15 pages in length) by the Provider Law

Firm per month for a Ride Share and Delivery legal issue. This

includes any combination thereof, but no more than 3 per

month. Additional for a flat fee of $75 per call, letter, or

review.

• Traffic Services. Defense of the Member for covered Ride

Share and Delivery moving violations in the court of original

jurisdiction (often called trial court) . These violations must

have happened while engaged in Ride Share and Delivery

activities. If your violation resulted in a misdemeanor or a

felony, you may choose to utilize the 25% Discount.

Plan Benefits:

• IRS Audit Services. The Member will receive IRS legal

services described under your Plan "IRS AUDIT LEGAL

SERVICES" for items included on a Schedule C and a

Schedule E of your personal federal income tax return

pertaining to Ride Share and Delivery income.

• 25% Discount. The Member will receive a 25% Discount of

the Provider Law Firm's corporate hourly rate for all other

legal work or matters beyond this coverage. If the Member is

sued for a Ride Share or Delivery service business related

matter, the Member may receive representation at a 25%

Discount for items that are not considered covered under

this Supplement. The Member will be entitled to the 25%

Discount as set forth in the Plan for Ride Share and Delivery

matters.
