ASCC Travel Checklist
The following must be provided as a complete packet at least 45 days prior
to travel. All forms/packets are available under ‘Forms’ on our website:
http://cms.cerritos.edu/activities/.
Please note: incomplete packets will NOT be accepted.
Student Eligibility Form (this form should be submitted BEFORE submitting the completed
ASCC Student Travel Request Form
Copy of Conference and Travel Request Form for Advisor(s) with all signatures
o All forms must be signed by advisor(s), manager and Vice President. No exceptions!
o Travel out of state, over $1,000, or to occur less than 30 days from time form
submitted require signature of the President/ Superintendent.
o Travel out of country requires Board of Trustee approval.
Student Travel Packet for each student traveling (liability forms)
Provide a printed copy of all travel arrangements (as applicable):
o Flight
o Shuttle
o Registration
o Lodging
o Map(s) for Mileage
o Parking
o Other ___________________________________________________
Claim statement for meal money, if applicable
o Show the meal money math breakdown on the statement
EXAMPLE: 5 participants @ $42/day = $210 x 3 days = $630
Maximum Meal Breakdown:
Breakfast: $10
Lunch: $14
Dinner: $18
o Submit all original receipts and any remaining meal money cash after the
trip to the Payroll Window
Receipt(s) must show payment method used: Check, Cash, or Credit Card
packet)
*Students must have paid Student Activities fee prior to submitting Student Eligibility Form (proven by
showing current semester sticker on student identification card)
