Please fill in the blanks on this form as applicable. You may mail the form and
payment to City Hall (505 3
rd
Street), drop off in utility drop box or send by email. If
you have any questions, please contact 715-386-4774 ext. 113. You will need to
include a copy of your CURRENT BOAT REGISTRATION INFORMATION. The sticker
and information form will be returned by mail to the address noted on the form.
The boat launch season pass sticker you are receiving is being issued for
identification that the launch fee has been paid. The sticker is for the current
boating season only. Issuance of a sticker does not guarantee that parking will be
available. A sticker for each boat or watercraft is required.
Please put the sticker on your boat trailer up near the tongue on the driver side of
the vehicle. If the boat launch (1

Street) parking lot is full, boaters can still launch
but cannot park their empty boat trailer on any public street or other city-owned
parking lot per City Ordinance.
