This letter is served upon you pursuant to a breach of your __________________ Non-
Disclosure Agreement, which has been in effect since the ____ day of ___________________,

It has come to our attention that you have used or disclosed Confidential Information in clear
violation of Article ___, Section ___ of your Agreement.
Accordingly, we demand that you immediately desist from further using or disclosing
Confidential Information and confirm in writing within twenty-four (24) hours your intent to do so.
We require that you provide us with the following information and documents to confirm your
compliance with this demand:
